Annual Comprehensive Physical Exam
Sixty (60) minute annual comprehensive physical exam during a visit with a focus on disease prevention and life-style/health improvement.
Annual Wellness Plan
An Annual Wellness Plan designed and reviewed with the Member by the Member’s PCP with follow up based on the Member’s individual needs.
Services covered that may be requested or medically necessary:
- School physicals
- Sports participation exams
- Nutrition counseling
- Exercise program review
- Routine vaccinations
- Primary care management in coordination with specialists of chronic diseases, including but not limited to: diabetes, chronic kidney disease, etc.
- Primary Care management in coordination with specialists of the treatment for hypertension, arthritis, asthma, COPD, osteoporosis, depression, or anxiety
- Primary care management in coordination with specialists of the treatment of other acute ailments, including but not limited to, respiratory infections, headaches, musculoskeletal injuries, rashes, skin infections, and urinary infections
- Provision of office procedures such as: ear irrigation, EKG, spirometry, bursa/joint injections, aspirations, wart removal, skin biopsies (shave, punch, and excisional), cyst/lipoma removal, and uncomplicated laceration repair
- Certain non-complex blood and urine tests. Tests which need to be sent to outside laboratories will be billed separately

Same or Next Day Appointments
Same or next day urgent care appointments with the Member’s PCP if such a request is properly communicated to the DPC staff and deemed medically necessary by the Member’s PCP.
Longer Visits Duration
PCP visits scheduled for no less than thirty (30) minutes.
After Hours Access
After hours access to PCP via cell phone and/or text for urgent unscheduled calls and communications.
Email Access
Access to PCP via email. Email messages shall be responded to as soon as is possible by the PCP.
Home Visits
Home visits upon reasonable request for an additional fee.
Negotiated Discount Prices
Access to negotiated discount prices for labs drawn in-house and on imaging procedures at specified partnered facilities.
