Direct Primary Care Fees for Gold Direct Care Members
The Services listed shall be rendered through a maximum of twelve (12) Visits per year to Members whose Employer tenders the applicable Monthly Fee as described below. The Member shall not be subject to any billing for co-pays for Visits.

The one-year period for purposes of measuring the maximum allowed visits shall run from the first date of a signed Agreement (the “Start Date”) and on each anniversary of the Start Date. The Monthly Fee must be paid on time for all Members and without interruption; otherwise, the Agreement will lapse and may be terminated by the DPC Practice as described in the Agreement. A lapse in the Agreement will result in the termination of the patient–physician relationship between the Member and the Physician.
Additional Charges
For each visit in excess of 12 per contract year (“Extra Visit”), the individual Member shall be additionally charged and obligated to pay $25 per Extra Visit. The PCP shall render, upon request by the Member, a home visit that is reasonably requested in advance for an additional charge of $30 if the home visit is located within 3 miles of the DPC Practice office, and $50 if the home visit is located beyond 3 miles of the DPC Practice office. Furthermore, the PCP shall render, upon request by the Member (or the Member’s legal surrogate), a visit for a Member residing in a long-term care facility, skilled nursing facility, or assisted living facility, for an additional charge of $100, which is the responsibility of the individual Member.
Laboratory processing fees are offered at a discounted cash rate payable by the Member to the DPC Practice. Patients may opt to send laboratory specimens to a local hospital for processing, in which case the hospital system will then seek payment from the patient’s insurance plan.
Gold Direct Care does not directly charge insurance for any services provided, nor does Gold Direct Care participate in or use coded billing for Membership Fees or Services. Patients are not prohibited from seeking insurance reimbursement, but doing so is the sole responsibility of the patient. The DPC Practice will provide invoices as created within our billing system; no special invoicing will be performed.
